What you’ll be part of
Garner is on a mission to transform the U.S. healthcare system — and we’re the only proven player doing exactly that. We partner with employers to redesign how healthcare works: applying 550+ proprietary clinical metrics across 80+ specialties to a dataset of 320M+ patients to identify the best-performing doctors, then using compelling incentives to steer members to the care that helps them get healthier, faster.
The result is a rare “win win” — better care and lower costs for both members and employers. In just five years, our work has helped over 2.5 million people access higher-quality care and saved $1B in healthcare costs. We recently raised our Series E and have doubled five years running. If you've ever wanted your work to solve a problem that touches every person in this country, this is the opportunity to do exactly that. You'd be joining a team fundamentally reimagining healthcare in the U.S. — and using AI to scale that impact further and faster than anyone else can.
About the role:
We are seeking an exceptional
Staff Software Engineer
to join our organization. Garner’s Engineering team tackles incredibly hard technical problems for a mission that truly matters. Our systems analyze billions of medical records to rank healthcare providers across the country. We are an AWS + Kubernetes shop, and we are constantly introducing new technologies as our system load doubles every year. We expect our engineers to champion cutting-edge industry practices as we aggressively embrace AI in every facet of our operations and norms. We are seeking teammates who take pride in delivering their best work while fostering a strong team culture.
Where you will work:
Garner is headquartered in NYC, but this position is available for individuals who are comfortable with remote work and occasional travel to HQ.
What you will do:
Lead team-level technical strategy and execution on multi-quarter time horizons
Use AI to build, optimize, and maintain software systems
Uplevel engineers across the organization by raising the technical bar
Collaborate across disciplines to understand our domain and iterate on projects
Protect our users’ privacy and security through best practices
The ideal candidate has:
6+ years of software engineering experience
Experience leading teams of engineers with demonstrated impact on technical strategy
A generalist software engineering mindset ready to learn new languages, frameworks, and techniques required for a fast paced startup environment
Demonstrated ability to build complex systems using AI coding agents (we use Claude Code)
Able to learn quickly and mentor others
Familiarity with data security and HIPAA compliance
A desire to be a part of a high-performing, mission-driven team that operates with intense urgency, a strong sense of individual accountability, and a commitment to authentic feedback
What you’ll get here
You’ll work on problems that matter, at a company working to change healthcare at scale. You’ll work at the intersection of AI and systemic healthcare reform, where the problems we solve are as interesting and compelling as the mission.
At Garner, you’ll take on real, ambitious problems with real ownership and autonomy, alongside exceptional, principles-based people who genuinely want you to win. It’s demanding by design. You’ll be challenged to stretch beyond what you thought possible and receive consistent coaching to help you grow and do the best work of your career. This isn't the right fit for everyone, and that's intentional. The people here are driven by what's at stake for real people, and that's what gives our intensity its purpose.
Technologies we use:
Claude Code, Kubernetes, AWS, Airflow, DBT, Python, TypeScript, React, Go
This is a unique opportunity to join a fast-growing company in a transformative role, helping shape the future of healthcare.
Compensation Transparency:
The target salary range for this position is $275,000 - $300,000. Individual compensation for this role will depend on various factors, including qualifications, skills, and applicable laws. In addition to base compensation, this role is eligible to participate in our equity incentive and competitive benefits plans, including but not limited to: flexible PTO, Medical/Dental/Vision plan options, 401(k), Teladoc Health and more.

MEET THE TEAM
Here at Cleo, we have ~200 Engineers organised into Pillars ✨ Our stack spans a Ruby on Rails monolith, a single React Native (TypeScript) app frontend, Python for machine learning services, and PostgreSQL, all hosted on AWS and shipped through Kubernetes - backend goes out multiple times a week, app releases to Google and Apple at least weekly. To thrive here, our engineers proactively use our Engineering Principles to guide daily decisions, take ownership of problems beyond their own squad to drive broader impact, and actively drive their own learning, development and growth.
ABOUT THE ROLE
We’re looking for a brilliant Frontend React/React Native Engineer with an eye for UI/UX design to join us on our mission to fight for the world's financial health. You’ll be joining a team of adaptable, creative and product-focused engineers who ship working software. We understand our customers, we understand their pain, and we are passionate about helping them.
Key responsibilities include:
- Working on our React Native application, building out amazing experiences for our users which bring financial health to life in the unique Cleo tone of voice.
- Writing automated tests alongside your code to give us the confidence to ship it
- Using AB-Tests, feature flags and other tools that let us iterate quickly
- Using data to dig into user journeys, detecting problems and helping to optimise the Cleo experience
- Being part of the rota for our weekly app releases to the Apple and Google app stores
- Getting involved in cross-cutting concerns that lift our entire engineering effort with the rest of the frontend chapter
- Taking part in shaping all the work your squad does, not just the technical parts. Delivery is a team sport, and we encourage everyone at Cleo to share their ideas, so you can expect to be involved in product ideation sessions, user research calls, design reviews, retrospectives, ways of working sessions, product demos, OKR reviews.
Whichever squad and part of the business you land in, you will be shipping changes multiple times a week to our hundreds of thousands of active users and seeing your work having a material impact on the financial health of those most in need.
WHAT WE'RE LOOKING FOR
You are passionate about making a positive difference in society by improving the financial health of our users. You align with our company values https://web.meetcleo.com/life-at-cleo and engineering principles https://web.meetcleo.com/blog/cleo-engineering-principles, which drive our ways of working and software delivery.
For this frontend focused role, we also have some specific asks:
- We’re looking for someone with strong industry experience of using React Native or React with TypeScript for a minimum of 2+ years.
- As your work will primarily involve working on features for our mobile apps, we’d either like to see some proven experience in this area or a genuine passion for moving into the mobile app space.
WHAT WE OFFER
We offer benefits package built to support you in and out of work. Benefits vary by country 🌍 and include meaningful equity, comprehensive health insurance, mental health support, a paid one-month sabbatical after four years, a learning & development platform, pension or retirement contributions, and location-specific leave entitlements.

We can't do that without building a brilliant, genuinely diverse team - and making sure our hiring process gives everyone a fair shot, running a fair and transparent recruitment process in which every candidate is considered on their merits.
We're glad to make reasonable adjustments at any stage of the process. If there's anything we can do to support you in showing us your best, please let your recruiter or one of the team know.
We may use AI-assisted tools during the recruitment process to take support the team in different ways like taking notes or referring back to conversations. They do not replace the team and all applications are reviewed by a Cleo employee thoroughly!

ABOUT THE TEAM
The Release Team owns the safe, continuous delivery of ClickHouse Cloud, a managed database platform running tens of thousands of ClickHouse clusters. We are responsible for upgrading and maintaining those clusters at scale, building the internal tooling that makes it possible, and being the last line of defense when something doesn't go according to plan.
ABOUT THE ROLE
This role is an equal split between operational execution and software development. You are responsible for the operational side: coordinating and running upgrades, dealing with edge cases that don't fit the happy path, and keeping tens of thousands of clusters healthy in production. At the same time, you are building and constantly improving the systems that make the next rollout safer and more automated than the last. If you find satisfaction in both writing the playbook and executing it, including the messy parts, this role is for you.
WHAT YOU'LL DO
- Plan and execute rolling upgrades across tens of thousands of ClickHouse clusters, ensuring safety, correctness, and minimal customer impact
- Own the full release pipeline: from pre-upgrade validation and staged rollouts to post-upgrade monitoring and incident response
- Investigate and resolve production issues as part of a regular on-call rotation, including snowflake clusters and edge cases that automation can't yet handle
- Build and improve the internal tooling and automation that makes large-scale database operations reliable and repeatable
- Work closely with the core database and cloud infrastructure teams to identify operational pain points and turn them into solved problems
- Support and educate other engineering teams using our internal tools
ABOUT YOU
- 5+ years of experience operating stateful distributed systems in production, such as databases, message queues, or storage systems
- Hands-on experience running upgrades or maintenance operations on live production data stores, at scale
- Strong production debugging skills; you are comfortable digging into unfamiliar systems under pressure
- Experience with cloud infrastructure (AWS, Azure, or GCP) and Kubernetes
- Software development experience in Go (or strong experience in another language and genuine willingness to learn)
- Experience with ClickHouse preferred (as a user, operator or contributor)

An overview of this role
As a Senior Backend Engineer with the
Database Excellence group
, you'll build and evolve the data frameworks and tooling that keep GitLab's application datastores scalable, healthy, and safe across both GitLab.com and thousands of self-managed instances. A primary focus for this role is building proactive data growth controls for GitLab.com, including retention and lifecycle management frameworks, enforcement of retention policies at schema-change time, and tooling that keeps database growth sustainable as AI-driven development accelerates how quickly data accumulates. You'll help teams across GitLab reason confidently about data architecture, placement, and lifecycle management, while proactively identifying saturation points in our GitLab.com infrastructure and driving corrective actions with partner teams. With the team's recent move into the Data Engineering organization, you'll also shape new scalability and data health features for self-managed customers, working hands-on with PostgreSQL and Ruby on Rails in a fully remote, highly collaborative environment.
Some examples of our projects:
Data Growth Controls (retention enforcement and data lifecycle management)
SQL Traffic Replay Tooling
Background Operations Framework
What you'll do
Develop and iterate backend features and data frameworks that make it safe and efficient for GitLab teams to work with data at scale across SaaS and self-managed deployments.
Design and build data retention and lifecycle management frameworks, including retention policy enforcement, bulk data removal tooling, and automated identification of tables that need retention policies.
Build shift-left guardrails, such as static analysis checks and schema-change-time enforcement, that catch unsustainable data growth patterns before they reach production.
Collaborate closely with product management, UX, frontend, infrastructure, software delivery, and analytics teams to design and ship high-performing solutions
Review and improve database-related changes from other engineers and community contributors, ensuring data integrity, safety, and performance across all deployment scenarios.
Design, build, and maintain tooling such as SQL traffic replay and background operations frameworks to proactively surface and address scalability and performance issues.
Research, design, and implement improvements to data retention and lifecycle management, database performance, scalability, and data health, including areas like soft delete strategies and database migration testing.
Document database best practices, patterns to avoid, and data architecture guidance to enable developers to make informed decisions.
Develop proactive tooling and guardrails that help developers detect and remediate potential performance and data issues early in the development lifecycle.
What you'll bring
Professional software engineering experience working with PostgreSQL in large, complex production environments, including high availability solutions with automatic failover and zero-downtime data migrations as well as managing large or fast-growing datasets through approaches such as data retention, archiving, partitioning, or lifecycle management.
Proficiency with Ruby on Rails or another Ruby framework, including designing, implementing, and reviewing backend features.
Ability to reason about software design, algorithms, and performance trade-offs at a system level.
Strong written communication skills and comfort working asynchronously in an all-remote, distributed team.
Self-directed work style with effective organizational skills and the ability to manage priorities as a "manager of one".
Alignment with GitLab's values, including transparency, collaboration, inclusion, and contribution to open source.
Openness to learning and applying new tools and approaches, including transferable experience from related data, infrastructure, or developer tooling domains.
Willingness to participate in our
tier 2 dabase on-call rotation
in your time zone.
The
Database Excellence Group
is a remote, agile backend team that builds and maintains the frameworks, patterns, and tooling that enable GitLab teams to work confidently with data at scale across all application datastores. As part of the broader Data Engineering organization, we focus on data scalability, health, and developer enablement for both GitLab.com and thousands of self-managed deployments, collaborating closely with infrastructure, software delivery, and analytics stages to ensure data changes are safe, performant, and compatible across diverse environments. We are independent, self-organized contributors, and we collaborate asynchronously across regions. We are currently expanding our mission from proactively identifying and addressing saturation points in GitLab.com infrastructure to also delivering scalability features and data health initiatives to self-managed customers.
